Atlantis conus abutment –Treatment of maxillary edentulous patient with one previous placed zimmer implant

      A 67-year-old male presented with a chief complaint that “My upper teeth look so bad’. A thorough examination showed that maxillary teeth required removal due to secondary caries and general bone loss. A Zimmer implant has been placed before on site #5.

      After treatment planning and discussion, the patient consented to extraction of six maxillary remaining teeth, removal of the crown on existing implant, followed by placement of additional 4 implants supported Atlantis Conus abutments. Interim denture was provided to determine the restorative space and Vertical Dimension of Occlusion.

Figure 1: Pre-treatment radiograph shows six remaining unrestorable teeth and one Zimmer implant supported Crown (4.1 mm X 11.5 mm).

Figure 2: Simplant software was used to plan optimal implant locations.

Figure 3: Four additional compatible Zimmer implants (4.1 mm X 11.5 mm) were placed with Healing Caps at site #7, 9, 11 and 12.

Figure 4: The working cast was fabricated by fixture level impression with border moldering.

Figure 5: The accuracy of impression was verified by multiple interlock puzzle pieces with engaging impression coping on working cast and tested intraorally according to previous article [1].

Figure 6: The dimension of Atlantis Conus abutments was confirmed by using plastic matrix both extraorally and intraorally according to the manufactory instruction.

Figure 7: Cobalt-chrome framework was fabricated on the master cast with abutments and SynCone Caps.

Figure 8: Final wax-up was done and tried intraorally

Figure 9: One SynCone Cap was processed in the denture during fabrication, the other four SynConeCaps were picked up intraorally by EZ Pickup Material (Sterngold).
